Slide Contact Form mod

The Slide Contact Form mod is programed to give the user an easy access to the contact form for questions and feedback. Contact button is always visible on the left side of the screen. When clicked on it will slide open the contact form without the need to exit the current page your on. After form is successfully submitted, the contact button will temporary disappear until page is refreshed again.
